Who this policy covers

This policy applies to physicians and other healthcare providers who submit an application or expression of interest to join our provider network (“you,” “an applicant”). It does not apply to patients — this site and the application process it describes involve no patient care, and we do not collect or process patient health information.

About the nature of the information we collect: the information described below is about you, the applicant, as a professional — your name, contact details, and credentials — not about any patient’s health condition, treatment, or care. It is professional and business information, similar to what appears in a public provider directory, not what’s commonly understood as “protected health information.”

Information we collect

When you apply, we collect:

  • Your full name
  • Your email address and phone number
  • Your practice address (street, city, state, ZIP code)
  • Your National Provider Identifier (NPI)
  • Your medical specialty
  • Your state medical license number, if your application requires additional review
  • Your confirmation (with a timestamp) that you are not subject to a federal healthcare exclusion and that the information you’ve submitted is accurate, if your application requires additional review

We may also collect the following, depending on how our application evolves:

  • Your professional website
  • The insurance plans you accept
  • A copy of your medical license (document upload), for verification purposes

We do not collect your Social Security Number.

We also generate some information ourselves as part of processing your application:

  • Verification results from checking your NPI against a national provider registry and your name against a federal healthcare-exclusion list
  • The IP address associated with your device when you complete the confirmation described above
  • An application reference number, so you can check your status without resubmitting your information
  • Internal notes made by our staff if your application is accepted, for follow-up purposes

How we use your information

We use the information we collect to:

  • Verify your credentials. We check your NPI against a national provider registry to confirm you are a real, active, licensed provider, and we check your information against a federal healthcare-exclusion list.
  • Review applications that need a closer look. Some applications require additional review — for example, if information doesn’t cleanly match during verification. In that case, we may ask you to confirm your license number and complete a brief confirmation about the accuracy of your application.
  • Communicate with you about your application, including its status and any follow-up we need from you.
  • Follow up with accepted applicants about next steps and onboarding.
  • Let you check your application status using the reference number we provide.

We do not use your information for advertising, and we do not build advertising profiles from it.

How we share your information

We share information only in the following circumstances:

  • Credential and exclusion verification. We check your NPI against a national provider registry and your information against a federal healthcare-exclusion list. These checks are performed against government-maintained sources for the purpose of verifying your identity and credentials and screening for healthcare program exclusions.
  • License verification (planned). As we roll out license-document verification, we may share your submitted license information with verification sources — including, where available, free public state licensing-board lookups, and otherwise a third-party verification service — solely to confirm your license is genuine and current.
  • Service providers. We use vendors that help us run our systems — for example, hosting our application and securely storing submitted documents. These vendors only receive the information necessary to provide their service to us, and are not permitted to use it for their own purposes.
  • Internal use. If your application is accepted, your information may be shared internally within our organization — including with the specific provider network or product you applied to join — to complete your onboarding. This is an internal use of your information by us, not a disclosure to an outside party.
  • Legal reasons. We may disclose information if required by law, or to protect the rights, safety, or property of applicants, our organization, or others.

We do not sell your personal information, and we do not share it with advertisers or data brokers.

Cookies and tracking

We do not use cookies, analytics, or tracking technologies on the pages where you submit or check on your application. (Our internal staff tools use a login session to authenticate our own team members — that session is unrelated to, and never used for, applicants or visitors to our public pages.)

Data retention

We keep your information for as long as necessary to process your application and, if accepted, to maintain your provider record and communicate with you. If your application is not accepted, we delete your personal information and retain only a minimal record (an application reference number and its status) that does not identify you. We may keep information longer where required to meet legal obligations.

Your privacy rights

Depending on where you live, you may have the right to:

  • Ask us what personal information we have collected about you
  • Ask us to correct inaccurate information
  • Ask us to delete your personal information, subject to legal exceptions (for example, we may need to retain some information to comply with the law)
