This site and the application process do not involve providing medical care to patients, and nothing about submitting or reviewing an application creates a clinician-patient relationship.
Eligibility
You may submit an application only if you are a licensed physician or other healthcare provider eligible to hold a National Provider Identifier (NPI), and you are submitting the application on your own behalf.
Accuracy of information
You agree that all information you submit — including your identity, contact information, National Provider Identifier, license information, and any other credentials — is true, accurate, current, and complete. If any information you provide is untrue, inaccurate, outdated, or incomplete, we may refuse, suspend, or terminate your application or your participation in our provider network, at our discretion.
Intellectual property
The content, design, and functionality of this site — excluding the information you submit about yourself — belong to us or our licensors and are protected by applicable intellectual property laws. You may not copy, modify, or distribute any part of this site except as necessary to submit your application.
Disclaimer of warranties
This site and the application process are provided “as is” and “as available,” without warranties of any kind, whether express or implied, including warranties of merchantability, fitness for a particular purpose, or non-infringement. Limitation of liability
